Visit www.Dgcustomerfirst.com during the contest time, use the survey code found on your Dollar General store receipt, fill out the survey, and provide your name, address, phone number, and email address.
- Entry By Mail
Send a card measuring 3 1/2 inches by 5 inches, labelled “Mail-In Entry,” to the following address: Dollar General Survey, PO Box 251328, West Bloomfield, Michigan 48325.
On the card, include your complete name (no initials), email address, street address (PO Boxes are unacceptable), city, state, ZIP code, phone number, and birth date in MM/DD/YYYY format.
